Comprehensive School Physical Activity Program Questionnaire (CSPAP-Q) v2.0.
The CSPAP Policies and Practices Questionnaire (CSPAP-Q) is a needs-assessment designed to help schools self-evaluate how their physical activity policies and practices align with best practices in the field. The survey covers practices related to the 5 components of a CSPAP. Respondents answer simple “yes” or “no” questions and receive feedback in the form of scores and grades for each component. The results can be used to plan and advocate for more robust school physical activity policies, practices, and programs.
The survey questions are presented in the following areas:
- Background information
- Wellness Policy Status
- Each component of a CSPAP
*Note: If you are unable to answer some of the questions, please make sure that you reach out to personnel within your school and/or district for support.
