We are interested in learning about your use of earphones/headphones when listening to music and your perceptions of ways to make listening safe.  This survey is for adults (18 years and older) and results will be shared with researchers at the University of Northern Colorado, the World Health Organization (WHO) and the National University of Singapore.

The survey will take approximately 5-10 minutes to complete.

 

The institutional review board of the University of Northern Colorado has approved this research.  Participation in this survey is voluntary. You may decide not to participate in this study and if you begin participation, you may still decide to stop and withdraw at any time. You can exit the survey by closing out of the browser window in which the survey is being hosted. Your decision will be respected and will not result in loss of benefits to which you are otherwise entitled. No identifying information will be collected in this survey. By completing this questionnaire, you will give permission for your participation.
