CONSENT FORM FOR HUMAN PARTICIPANTS IN RESEARCH UNIVERSITY OF NORTHERN COLORADO

Project Title: Maximizing Safety and Minimizing Trauma: Understanding Components of Trauma-Informed Lockdown Drills

We are conducting a study to obtain feedback about the components of trauma-informed lockdown drills. This study will gather data from current school-based crisis experts to create a trauma-informed lockdown drill protocol. 

This study has been approved by University of Northern Colorado IRB (IRB# 2205039462).

The purpose of this study is to obtain feedback about the current perceptions of lockdown drills, as well as possible improvements to current crisis mitigation protocols that are being used within U.S. schools. This study will gather information about current school-based practices such as: perception of current lockdown drill practices, attitudes towards crisis prevention strategies, and suggestions for changes to current lockdown drill protocols. The information gathered from this survey will help us better understand the components of trauma-informed lockdown drills and preferences of school personnel, in turn, will inform crisis prevention responses. There is no financial cost to the participant in this research survey. Estimated survey completion time will be 10-15 minutes.

Interested participants must:
- Be at least 18 years old to consent to participate
- Be currently employed at a school/school district AND have expertise in crisis prevention/crisis response
 - Fluent in English

Possible risks to you related to your involvement in this research survey and process are minimal. However, we recognize that the topic of lockdown drills and gun violence may be uncomfortable for some people.

Your responses are confidential. Emails will only be seen by the research team and used for the follow up surveys, and will not be distributed to anyone else for research or other purposes. The research team will not attempt to determine the identity of those who have participated in the study. The data collected in this study will be examined in aggregate and no individual data will be published. Participation is voluntary. If you decide to participate in this study, you may still decide to stop and withdraw at any time. Please take your time to read and thoroughly review this document and decide whether you would like to participate in this research study.
